INSURANCE:

• Dr. Gilbert is not contracted with any insurance companies and does not take ​any insurance as payment                                                                                                                              
  
• Payment is due at the time of each appointment

• Patients will be provided with a receipt to submit ​to their insurance company for any reimbursement available to them                                                                                                                                   
  
• Patients are responsible for their own potential reimbursement from any insurance company, and any inquiries to their insurance company regarding reimbursement.  Most insurance companies categorize the appointment as an out of network visit, but due to the variety of companies and individual insurance plans, each person's situation could be accomodated differently

     MEDICARE:

• Dr. Gilbert has opted out of Medicare but can see Medicare patients through a private contract between him and the patient                                             
• Please note that Medicare will not reimburse for these services                                                                                                                                                          ​
